The pilot of a small plane that crashed on departure from Catalina Airport last month, killing all five people aboard, ignored a warning from the island airport manager that the flight would be unapproved due to darkness.

A preliminary report reveals that the aircraft had insufficient power and needed to be charged shortly before takeoff. It also says the plane did not have clearance to take off.
